Attention all Madison Residents: Now is your opportunity to get involved!  The Madison Health Department encourages all residents to participate in our community health assessment.  The assessment will help us gather accurate information representative of our community and identify issues for action to help improve the overall health and well-being of our residents.  

Starting on February 1, 2018 and during the months of February and March, please fill out the online assessment.

Alternatively you can complete a paper copy at the following locations:

• Scranton Memorial Library, 801 Boston Post Road
• Madison Chamber of Commerce Office, 12 School Street
• Madison Senior Center, 29 Bradley Road
